Initial Visit
Before beginning any treatment, you will have a thorough examination and consultation with Dr. Lazar; He will explain the nature of your dental ailment and will recommend the treatment options that will best suit your needs.
Occasionally, Endodontic treatment can be done the same day as the consultation. However, a complex treatment plan may require an evaluation and an appointment to provide treatment on another day.
Please provide the following information at the time of your consultation:
Your Endodontic referral slip and any X-rays if applicable
A list of medications you are presently taking
If you have dental insurance, bring the evidence of coverage or the necessary completed forms.
This will save time and allow us to maximize your plan benefits.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation.
Please alert the office if you have a medical condition that may be of concern prior to your endodontic treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, Pregnancy, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
Please give at least 24 hours notice of change of appointment if cancellation is absolutely necessary, otherwise a charge may be applied.
