Prime Endodontics is a preferred provider for most Union Plans, PPO, and HMO dental plans; often the out of pocket costs are insignificant.
Please understand that the total payment of the fee for dental services by Prime Endodontics is your responsibility and not that of the insurance company. As a courtesy, insurance forms will be completed without charge.
Please bring your dental insurance information with you so that we may have the correct billing information. Bringing your insurance information with you will help expedite the filing of your claim.
We want to provide you with quality endodontic care and the most convenient financial options. We can accomplish this by working closely with you to maximize your insurance reimbursement for covered procedures. We allow dental insurance companies two months to remit payment to us. If you have any questions about insurance or your initial visit, please ask our office staff. They are well informed, up-to-date and will make every attempt to assist you.
